When you request an appointment at Marked for Grace Counseling Center, we will ask you to provide your name, date of birth, telephone number, and e-mail address. This information will be used to establish you as a client and a chart will be created. We will maintain a record of your information at the office of Marked for Grace Counseling Center for billing and communication purposes. The employees of Marked for Grace Counseling Center are each required to sign and acknowledge a Confidentiality And Non-disclosure Agreement. Each employee has been instructed on maintaining the privacy of each customer and the importance of protecting the customer’s personal information.

Services will be paid for by credit card which is kept on file in a secure electronic health record (EHR). As a client you have access to your information stored in the EHR and can update when needed. A statement of what has been charged will be provided on request.

When you send us an e-mail from this website, you will provide us with certain personally identifiable information including your e-mail address.
